Our practice provides thoughtful, comprehensive primary care with integrated mental health support for adults and seniors. We believe health is best addressed when the whole person is considered—physical well-being, emotional health, life transitions, and the realities of aging.
We specialize in caring for adults and older adults who value a more personalized, relationship-based approach to healthcare. By integrating mental health into routine primary care, we are able to address concerns such as mood changes, anxiety, cognitive health, stress, and adjustment to medical or life changes in a way that feels natural, respectful, and collaborative.
Our approach is especially suited for seniors and their families who are seeking care that goes beyond symptom management. We focus on maintaining function, independence, and quality of life while providing clear guidance and support through complex medical and emotional decisions. Care is never rushed, and listening is central to everything we do.

We provide comprehensive adult primary care with an emphasis on prevention, diagnosis, and longitudinal management of acute and chronic medical conditions. Services include health maintenance and preventive screening, management of hypertension, diabetes, hyperlipidemia, cardiovascular and respiratory conditions, medication management, and coordination of specialty care when indicated.
Mental health assessment and support are integrated into routine primary care visits, allowing for early identification and management of conditions such as depression, anxiety, sleep disorders, and stress-related concerns within the context of overall medical health. Care is evidence-based, thorough, and focused on continuity and clinical oversight.

We offer geriatric-focused primary care addressing the complex medical and functional needs of older adults. Care includes comprehensive medication review, management of multiple chronic conditions, cognitive assessment and monitoring, evaluation of functional status, and guidance through age-related health transitions.
Our approach emphasizes safety, quality of life, and appropriate medical decision-making, with attention to how medical conditions, mental health, and social factors interact in aging. Collaboration with families and caregivers is incorporated when appropriate to support informed and coordinated care.
Telehealth visits are available for select primary care and mental health needs, including follow-up care, medication management, symptom review, and care coordination. This service allows patients to maintain continuity of care when in-person visits are not feasible while preserving clinical oversight and accessibility.
Telehealth is utilized when clinically appropriate and integrated into the overall care plan to ensure consistency and safety.

Michelle Rentas, DNP, APRN, FNP-BC, is a board-certified Family Nurse Practitioner providing comprehensive, relationship-based primary care for adults across the lifespan. She earned her Doctor of Nursing Practice degree from the University of Florida and is committed to delivering high-quality, evidence-based care that addresses both physical and mental well-being.
Dr. Rentas offers full-scope primary care, including preventive care, chronic disease management, and care coordination, with an integrated approach to mental health. She believes effective primary care goes beyond treating symptoms—it requires understanding the whole person, including how stress, mood, cognition, and life transitions impact overall health.
In addition to general primary care, Dr. Rentas has advanced expertise in geriatric medicine and the care of patients with complex medical and cognitive needs. She works closely with patients and families to manage conditions such as memory changes, dementia, and behavioral symptoms related to neurocognitive and other mental health conditions. Her approach prioritizes thoughtful evaluation and conservative treatment, avoiding psychiatry unless it is clinically necessary to support safety, function, and quality of life.
Known for her attentive and compassionate style, Dr. Rentas partners with patients and assisted living faciilties to create individualized care plans that are practical, respectful, and aligned with each patient’s goals - even at the comfort of their home, Whether caring for adults seeking a trusted primary care provider or older patients requiring more specialized support, she is dedicated to helping patients feel heard, supported, and confident in their care.

Edwin Rentas, Jr. serves as the Chief Operating Officer (COO) of Rentas Advance Medical, where he oversees the organization's strategic and daily operations. He is the driving force behind the practice's financial stability, technological advancements, and operational excellence, all while ensuring high-quality, integrated primary and mental health care, particularly focusing on geriatric mental health and compassionate care for seniors. His leadership is essential for the practice's sustained growth, commitment to patient satisfaction, and the effectiveness of mental health assessments, including those provided in at-home mental health settings.

Katie Bell holds a Bachelor of Science in Nursing and possesses over eight years of clinical experience, demonstrating a long-standing commitment to patient care. She is deeply passionate about preventative care, particularly in the realm of geriatric mental health, and promotes a holistic approach to healing that nurtures both the mind and body. With her expertise, she is dedicated to providing compassionate care for seniors, including conducting mental health assessments and supporting at home mental health initiatives.
